The General Pharmaceutical Council of Spain (CGCOF) and the Spanish Society of Hospital Pharmacy (SEFH) have presented the “Juntos” project, which aims to improve pharmaceutical care for patients with solid organ transplants. The initiative has the backing of the National Transplant Organisation (ONT) and six patient associations. Although Spain is a world leader in transplantation, the estimated proportion of transplant recipients with non-adherence to their immunosuppressive treatment ranges from 20% to 54%. In order to seek an innovative approach and promote effective measures to encourage, among others, adherence to treatment in transplant patients, community and hospital pharmacists are joining forces in this healthcare initiative
